I consent to participate in the WholeLife programme, run by the Coastal Primary Care Network. I understand that my participation in this programme is voluntary and that I am free to withdraw at any time, without giving any reason, and without my medical care or legal rights being affected. I understand that as part of this lifestyle intervention I will need to undertake baseline and post-programme blood tests/measurements to evaluate my changes on the course.

I understand and give consent for the Wholelife project to access my GP medical record and that my GP will be informed of my participation in the programme. I understand and give consent for data, including relevant medical and health information, gathered before and during the WholeLife Project and as part of the post-programme evaluation to be shared with individuals outside of The Coastal Network PCN. Any data shared with individuals outside of The Coastal Network shall be anonymised.
